Study on childhood obesity

WHAT'S THE PURPOSE OF THIS STUDY?
A research study to survey teachers', parents', and
therapists' perspectives on childhood obesity
is being conducted. The purpose of this survey is to examine
current perspectives on childhood
obesity from occupational therapist, physical therapist,
teacher, and parent points of view. The
survey will touch on the prevalence of childhood obesity,
needs of children with weight issues,
and current attitudes about childhood obesity from different
perspectives. The hope is to gain
insight into children's weight issues in different contexts:
in therapy, in the classroom, and at
home.
WHAT DO I HAVE TO DO?
You will complete a survey online. It will take about 30-40
minutes to complete. The survey
includes questions about occupational therapy treatment,
physical therapy treatment, classroom
concerns, and home experiences of children with weight
issues. However, you will only be asked
to complete the part ofthe survey that is relevant to you
(Le. occupational therapist, physical
therapist, teacher, or parent).
ARE MY RESPONSES CONFIDENTIAL?
The survey data will be treated confidentially and may not
be disclosed, unless required by law or
regulation. Survey data will be stored in locked files. You
will be assigned a participant number,
which will be the only link to your survey responses. This
means that the data will be conducted
anonymously. Only researchers will see your individual
survey responses. At the end ofthe
survey, you will be asked for permission to use quotations
ONLY for professional presentations
and publications Ifyou agree, no names or nicknames will be
used.
