Yes, but one must download the Acrobat Reader. Some people don't like
to have to download something, or may be afraid to. Save your word
processed files in an .rtf format (rich text file). Whatever word
processing app the user has will open it - cross platform.
I used to make everything as a .doc and a .pdf for posting on my class
web page. That one .rtf file type covers everything very quickly and
simply.
As far as creating an interactive form, how savvy are you with coding
your web pages? If you use a WSYWIG (Dreamweaver, FrontPage) there
should be a way that you can build one there and then fill in your
return variables - check your help files. If you are a hand coder, you
can find form templates and scripts online by doing a search - there
are many, many ways to do what you're wanting so don't stop looking
because it looks too complicated! There are also plenty of interactive
sites that can create the code for you specifications.
As for running the script that will process and return the results,
you'll need to check with your server to see what scripts they provide.
They may even have an app that will build the form for you.
