Hmm, since you are the doing the creating then it gets down
to you having to create the appropriate form. As Neal said
the best alternative is to create a PDF. With Adobe Acrobat
you can actually create fields in your PDF form. The problem
here is that you need Acrobat. If it is a one shot deal, then
go to Adobe and download a demo. Otherwise, you may want to
buy it if you plan on using it a lot.
Acrobat is an excellent application. You can import
PowerPoint files, Spreadsheets, almost any document, etc. If
you plan on creating a lot of PDF then it is well worth the
money...you do get an educational discount.
I don't know if you can create PDF forms in NeoOffice/
OpenOffice which is free, or if you can do it in Word 2007. I
would say yes with the latter. But, if you get Word 2007 you
have to go to MS and download the PDF plug-in. I don't get
this MS move, but I rarely understand much of what MS does
anymore.
