I always put it on school letterhead, use a school envelop and
school stamps and mails it so I have never asked for that. My
last school had a "brag sheet" where students filled in school
activities, accomplishments, etc. And the other piece that
students often forget is the university address.
Personally I like sharing the letter with the student and
usually ask them to proofread it right before I send it off.
That way the student knows exactly what is going out and gets
to read all the wonderful things you write about them.
