    Okay...
    I would start out by typing up a list of my expectations.
    How should the kids come in the door? How should they be
    prepared? What should they bring to class? What shouldn't they
    bring? What is allowed and not allowed during instructional
    time? What about quiet work time? What are the restroom, drink,
    pencil sharpening rules? Homework expectations? Give everyone a
    copy and slowly review all of them. Tell them you expect them to
    keep this copy handy in their binder (front cover of their book,
    etc.) Check each day to see if they have it. This only takes a
    second to have everyone hold it up. If they don't, take points
    off their grade and give them a new copy. When someone is not
    meeting an expectation, remind them, "Joey, what expectation are
    you not meeting? Check your list...."
    Use some sort of system for people not meeting
    expectations...such as a time out. A certain amount of time outs
    is a call home or grade reduction or whatever works for your
    school.
    This approach really works for me. I am firm but fair and VERY
    consistent. I'm not known as the most "fun" teacher in my grade
    (I'm too old), but I have great management and we are able to do
    a ton of group projects and have unstructured activities because
    I have good control.
