You were here to ask if you should write her up and if others would write
her up. You didn't ask why writing her up worked last year but that it
did means it just might again this year. Which is amazing considering her
lack of impulse control - maybe she has some after all. But goodness why
wonder if you should do it if it worked - go for what works. Likely if you
write her as often as you did last year, you can bring her around again
little by little. History often repeats itself and so why doubt yourself
    so much?

And amazing that you were able to make headway with such a troubled
student - but the not looking you in the eye stuff might be cultural.
Kids from the islands - it's not appropriate to look an adult in the eyes
    - they're taught to keep their heads down in one on one conversations with
adults. I'd give her the benefit of the doubt on that one unless... she
used to look you in the eye, then I'd make certain sure she did it again
    and every time we talked.

Keep your referral slips close at hand and your pencil sharpened.
