I use the foldable where you take sheets of notebook paper and fold
it in half "hamburger" style leaving 2 lines showing at the bottom
of the first page, then two lines showing at the bottom of the
second page.... I can show this better than explain. Anyway, you
end up with a little booklet with tabs showing at the bottom of each
sheet. Each tab can be a topic, vocab word or a heading at the
beginning of a section in the textbook. For textbook reading, I've
had the kids label each tab with the heading and take cornell notes
inside the booklet on the page. They decorate the outside cover.
They can paste into their science notebook or keep it in a pocket,
or punch holes in it to store in their binder.

This foldable is really useful because you can keep adding pieces of
notebook paper to make additional tabs to match the number of
    sections in the reading selection.

Another good one is where they use fold the paper in half
"hamburger" style, then fold the ends into the center so they have 4
equal columns. If I have six vocab words, I would have them divide
the left-most and right-most column into three equal sections and
cut tabs. The tab folds into the center. A picture goes on the
front of the tab, the word goes on the back and the definition goes
    in the middle column.
