I would recommend creating a Google form (free using Google
documents) to collect and aggregate this data. I certainly
would be more inclined to answer these questions in that
way....you will find the data collection so much easier as
well. From someone knee deep in action research and data
collection...it's a wicked time saver. Also question 8 and 9
might need to be checked for bias? Just a friendly
suggestion.
