I haven't read any of the other responses so I hope this isn't
a repeat. In my district those accommodations are put in
the "Conference Notes" portion of the IEP. When my son was in
school (neighboring district) the middle and high schools had
a second accommodations sheet that had a list of
accommodations. We marked the accommodations he would need and
the teachers had to follow through.
