Can you tell me if the medical eval form is this one? https://www.opm.gov/forms/pdf_fill/of178.pdf It would be great to at least know what all has to be done so it can be scheduled, especially if you can't get it all done in one place.

Yes, it's that form but they had filled out the requirements of the job like lifting, working with others, etc

I called a random audiologist. It was just my basic copay of $20. Took all of 15 minutes tops. Google or search for audiologist in your area.

I had a fellow SLP in my school district do the hearing portion. We all have access to audiometers and are trained to use them. My HR person confirmed this was okay.
