So any recommendations on where to get the medical exams done for myself and family???

Ida Then I would definite go to a workplace clinic. They have all the extra equipment needed for the eye and hearing test.

SpEd_Eddie Having trouble finding a clinic willing to preform the exams for my family. Getting a lot of "we don't do that type of thing", and "don't you have a form to fill out. We need a form from your HR." Been on the phone for an hour.

Icanread My husband's doctor was out of town for a few weeks so he went to Walgreens to get the all-clear for travel. It took some calling around before we found a place he could go to. The PA just noted on the Walgreens form that he was healthy and cleared for travel. I took my kids to their pediatrician and they wrote a letter on letterhead.

Couple of Questions We DID receive a form from HR. It was really basic for me and the kids. Husband's was more in-depth. I can look to see if we have a blank copy still of the original for you.

HelpOthers If you still need this, contact HR and ask for the form. They actually do need to send it to you, because there is specific information they need for your position. Your family members just need a doctor's note (Walgreen's letter, etc.) stating they are fit to travel/live overseas. I had my form completed at the local military facility. As a civili...See More
