Leo You could order birth certificates and marriage certificates if you don't have those on hand. Your dependents will need a signed letter on the clinics letterhead from their doctor stating they are healthy to travel/live abroad.

Thankinghim Leo I am in this same situation. Do you know if there is a specific form for the kids to have filled out? Does it have to be their primary dr?I have tried but can't get an appointment befite middle of next month. Just want to know what I CAN DO KOW.

HelpOthers No form is needed for family members, just a note from medical. I took my kids for sports/school physicals at a CVS or Walgreens. My husband had a general physical completed. With their physicals, on letter-head, the doctors/nurse practitioners wrote a statement indicating that they were healthy enough to move overseas. They also had whatever shots...See More

Leo ^ What Mary said.. Getting the signed letters for dependents was very simple. The physical itself (for me) was also pretty straight forward.
