Is Italy the exception, or do most locations have similar rules about income from the US while overseas?

Totally fine. I owned a house in the states and rented it out while in Italy. People are not allowed to work on the economy, so spouses and dependents can't work unless it's on base. You are allowed to rent your property, don't read to much into "the rules" as they were written long ago. There are jobs for spouses and children. Mine worked as a para, USO, and kids were lifeguards and baggers at the commissary.
