    I use parent surveys when I sent home assessment/unit
    performance results. To make my life easy, I type up a
    generic letter telling what skills were covered. I then make
    a copy for each student/family and highlight the areas that
    student needs to work on. I also include a survey checklist
    titled "how can I help you help your child?" and include
    things like send extra practice pages home, provide websites
    for home, call/email you, etc. I also let parents know that I
    will continue to work with the child during small group time.

    Because it is individualized and an easy checklist, parents
    are motivated to respond. I've had a lot of success with
    checklist surveys based on individual students. I hope this
    example helps.
