I am writing an essay on how teachers should report child
abuse in Kentucky. I have to indicate the type of
documentation that teachers are required to fill out, but
can not find any on the school district websites I have
searched.

Do teachers have to fill out a specific written report, or
do they create their own? Is there any other
documentation they are required by the school district to
keep?
