You should immediately request assault leave, this will
allow you be paid while in your treatment facility. You can
not wait for a school or district to offer it, as they
likely will not. This is FAR better then using your
personal leave or FMLA which is essentially unpaid.

You need to contact your school and district quickly,
without contact they may decide that you have abandoned
    your position.

You should immediately report the assault to your local
city law enforcement, not the schools police division. You
are the victim of a crime, and you have rights.

The accusation of "talking about the incident" is
frivolous, short of a court order the district or school
    can not gag you.

Contact your professional association if you are a member
of one you have a right to another PDAS evaluater and
evaluation,. Unfortunately it is well past the time by now
for you to do much. Proficient is low for you but it isnt
    bad.

What were the specifics of the growth plan?
