Vision part of medical exam

So, I have yet to receive the actual health check form from HR, but I've been using a friend's copy of it. Its checklist says the only vision thing we have to get tested is long-distance vision. I've been able to get that done. The peripheral and color testing on form is another appointment and will, of course, cost more. Do we have to get those tests done as well, or will the long distance vision by itself be okay? I'm currently overseas, so trying to take advantage of my health insurance within my last few days here versus paying full non-U.S.-insured price when I go home next week (until my DOD move).

Nana B Urinalysis: On page 6 of OF 178 - Item h. - "if indicated." Like with everyone else on this forum it seems, there was no indication on my form that urinalysis needed to be done.

Teach I also did not get the urinalysis. My doctor said that there was nothing on the paperwork noting it was needed and he looked at my records and saw nothing that indicated he should have me do it. They really need to update the form so that it only has what we need! :)

Bett Okay, got the full eye exam completed yesterday --- just in case it's needed. The doctor asked if I was going to be a driver or something for all these vision tests. Good news is I passed, so lol guess I can also be a bus driver. I don't have urinalysis, and won't worry about it. So, now all I have to do is drop all of this off at HR in Alexandria ...See More

SandArt Very smart, Bett. I wish I had started before I got the forms. I got the forms exactly 2 weeks ago. I am sending them back tomorrow. However, if I had known some of the stuff sooner, I could have sent them back the same week.
