I believe if you have kids there is one step that has to be completed at a passport office. We had to go to get a form for our daughter filled out I believe.

Wilma is correct if under 16 the passport must be done in person with the child present.

And if your child(ren) are under 16, both parents need to be there in person, or the one present needs a notarized form (see earlier post) signed by the other parent giving permission. If there is only one legal parent, you need documentation to prove this.
