Component IV Knowledge of Students ELA-AYA

I have compiled the information from a survey I gave my students during the 1st week of school into a 1 page document. I am struggling to decide if this is a good piece of evidence for the component IV Knowledge of Students. I am wondering if I should stick to sources of knowledge outside of the students themselves when providing evidence. Thoughts?
