There is no eval form for department heads in my district. Formally they are evaluated using the same criteria as other teachers, and that evaluation doesn't take their dept head duties into consideration. The main thing that keeps a department head accountable to their department head responsibilities is that every other year the department head is either re-chosen to serve another two years or replaced, based on voting of the teachers within the department. Also if they are not fulfilling their duties from the perspective of administration (not attending meetings or not submitting paperwork or keeping records), administration can assign a different teacher to take over the position.

Our school rotates this position each year so that it isn't so overwhelming for any particular teacher. Maybe you can suggest that at your school.

If department heads evaluate or inform the evaluations of other teachers in the department, does this impact the ability to effectively collaborate with colleagues?
In my district per union contract a teacher can't participate in the evaluation of another teacher, so department heads do not have a role in evaluation. In part, this is for the reasons you stated.
I’m an elementary teacher who has developed a fun but effective math/music program for students grades 3-6. The audio and video series are aligned to State and Common Core Standards and really motivates students to learn math and equips them with the foundation to excel on Standardized TESTS.
