I need some reasons as to why an individual may not pass the essay of the general knowledge test. I was told that grammartical errors are the main reason is that true?

I don't think that's the reason. I was told it's based on whether or not you can develop an essay having a thesis statement, body and conclusion.

I took an FTCE General Knowledge essay class and the professor contributed the failure rate to weak essay structure, thesis statement and supporting details. I am working on strengthening these areas to pass the test.
