For the tentative offer the 306 (declaration) and 5013 (employment verification), they were fillable pdf documents. Kind of odd that the rest of the paperwork would be "worse copy ever" but I would not know yet. Thanks for the heads up on the medical evaluation. Is there some generic form for a doctor to fill out?

It was a leap of faith for sure. The good news is that very few people seem to crash out at this point, even with credit problems and fairly significant medical issues. The main thing with the medical is that you (or a family member) does not have a medical condition that requires medical treatment or medicine that is not available at your proposed...See More

While I had a tentative offer early last week and returned everything same morning filled out, I then got a call on Thursday morning asking if I was accepting or not.
