The medical evaluation came with the big packet after the firm offer.
No, spouses and children need something on doctor/hospital letterhead that says they are healthy enough to travel.

TeacherMama722, so it's a "pre-employment" exam? They have to fill out a form? I am going to see if I can schedule an appointment a month out, before the paperwork gets here. And the spouse and kids just need a generic letter?

Yes, not a full physical. Vision, hearing, blood pressure, etc.
