Hey! I am wondering what the exact Health Forms for new hires look like. Is it a generic health form? Is there just a direction that says "go get a physical"?

What about for dependents?

ALSO - have people seen that on the DoDDS FB group people who are getting their background checks are saying that they have been directed to authorize a HIPAA waiver for DoD or risk losing their jobs. Any one hear about that?
TeacherMama K in Daegu there is a specific form, it isn't a physical but a medical evaluation with mostly hearing, vision screenings. (so no bloodwork).

dependents need a signed letter from their physician saying they are fit to travel.

Happy Teacher Thanks, TeacherMama! That's not so ominous. No guide for the dependent letter? Just a letter signifying fitness for travel?

Euxpat Never heard anything about HIPPA and seems discriminatory to ask for a waiver. Unless they can do this to find out if you can't perform your job due to injury or have special medical needs that cannot be met due to location?

Happy Teacher Euxpat - the latest chatter on the board is that the requirement, if challenged, has been corrected by some HR offices. Unless there is a specific reason for it (return from FMLA to determine return to work, etc), it is not ok to ask for it.
