Just a few questions: 1. Does anyone know if we should fill out a separate form for each school at which we have worked? I worked in 2 different schools that were part of the same district. Should I list the district, or should I list the separate schools? 2. I also spent 2 years working as a substitute teacher in the same district (before I became a teacher, while I was completing school). Does this experience count as well? 3. If it does, should I submit a separate form for that as well, since my title is different?
BubbleHead As far as (1) goes send the Employment Verification form and a self address stamped envelope to the Human Resource Department. They will fill in the information for all your work experience in the district I sent EV to 3 different counties, it took about two weeks for 2 to get back and im still waiting on the third.

haha Only full time teacher of record experience counts. Agree that HR and/or district will/should take care of the form. As long as your years are right it doesn't matter for salary purposes.
