$4500 per year sounds about right for us (FEHB, high option, family).

This website has a tool. I am looking at a pay statement from last year (just to give a good idea). We paid $163.40 every paycheck (every two weeks) for FEHB which is the medical insurance (we use Foreign Service Benefit Plan). I have a family of 4.
