Do we send this form to our past admin/principals or to the district/HR we worked for ? Does the employer have to send it back through a sealed envelope or can they sent it through email?

Thank everyone for responding ! I went ahead and emailed usarmy.ria.chra-nc.mbx.dodea-eas-application-inquiries@mail.mil. They told me I can send the form to either Principal or HR. they also said that typically if you send the form to a principal they forward it to HR because they wont know the exact dates. @KMC I just tried to upload mine and it wouldn't let me either even though it was a .pdf. I was giving it the name "Employment Verification." It didn't give an error, just didn't add anything when you hit upload file. Then I tried by naming the document exactly the way it was listed in the drop down list and it then added it. Hope that helps.
