OregonSLP, yes, it was in the firm offer packet. You probably filled it out and forgot. I had to check my documents to be sure!

Oh that's a distinct possibility. The past two months has been such a blur, who knows what I've agreed to in all of that paperwork : )

Firm offer packet? So"there is another packet after the one with the physical W4 etc. and this state tax form is not in the first packet??

Mr. Fish, when I received my firm offer the email included the in-processing packet. Which had everything you mentioned plus the firm offer letter.

My big packet. And with my tentative offer. My firm offer I did not sign and return as my orders served as my firm. The direct deposit and tax forms are to be turned in to your secretary upon arrival -- they are your financial gurus
