Congratulations!! I can't help answer those questions. But would you mind sharing your timeline of when you interviewed and when you got the offer? I'm on day 6 of waiting after my interview.

If you google it, some version of both of those should come up. But, they should be sending you the forms or a link to the forms to make certain you are getting and submitting the correct/most recent version. When in doubt, keep writing your contact person at HR.
